Appointments are an important part of your treatment, as time has been reserved specifically for you. Please arrive on time for your appointment. If you are more than 15 minutes late, you may be asked to reschedule. We request a 24-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and we appreciate your cooperation.


Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Please inform us of any changes to your insurance coverage.


We never want costs to stand in the way of quality treatment, which is why we are dedicated to providing our patients with care that is both effective and affordable. Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. We offer a variety of payment options to meet your needs. Forms of payment accepted by the office are check, cash, or any major credit card. Financing options are available through CareCredit to help cover any out-of-pocket expenses. We will be more than happy to submit all PPO insurance claims for you and help you recover the most from your benefits. We will thoroughly explain your choices and work hard to accommodate you.
